Editorial summary

Hydration, sweating and subsequent injury answer different questions. Read the comparator and endpoint together before assigning a repair mechanism.

Evidence layer

One hydration reading, several possible explanations

A higher hydration reading can be a useful observation without establishing barrier repair. A diabetic-mouse intervention examined hydration alongside sweating; a small radiotherapy cohort related baseline measurements to later injury. They did not test the same product.

Effect of Mucopolysaccharide Polysulfate on Sweating Function and Skin Dryness in a Mouse Model of Streptozotocin-Induced Diabetes. · Relationship between severe radiodermatitis and skin barrier functions in patients with head and neck cancer: A prospective observational study.

In diabetic mouse footpads, mucopolysaccharide polysulfate cream produced higher sweat-droplet counts and surface hydration than its own placebo. In a separate comparison, hydration did not differ significantly from petrolatum or urea cream, while the sweat-droplet increase versus untreated diabetic mice was only a trend.

These comparisons cannot be merged into either “no effect” or “barrier repair through restored sweat glands.” Placebo, other moisturizers and untreated skin are distinct controls. Nonsignificance is not equivalence. Sweat-stimulation responses and measured nerve or vessel density also do not establish structural nerve repair.

Evidence layer

Similar surface readings, different subsequent outcomes

Nine patients completed the radiotherapy study and three developed severe radiodermatitis. Baseline hydration and transepidermal water loss did not differ significantly between severe and non-severe groups, whereas dermal echogenicity did. TEWL measures outward water loss; ultrasound provides a different tissue signal.

The small observational study cannot establish that moisturizers masked damage or that modifying echogenicity or microbiota would prevent injury. Concurrent treatment, moisturizers and topical corticosteroids limit causal interpretation. The subepidermal low-echogenic band is not a direct measurement of dermal–epidermal junction damage.

The mouse study connects hydration observations with sweating function; the patient study limits what baseline surface measurements alone can predict. Neither supplies the other study’s mechanism. Muzi would specify hydration, water loss and recovery from a defined insult as separate research questions.

Evidence layer

A practical next comparison

In an appropriately approved target study, compare the active formula with its matched vehicle, fix environment and measurement timing, and record hydration, TEWL and sweating conditions. Pharmacological sweat suppression is not proposed as a routine formulation-screening step.

Use independent batches and a prespecified recovery endpoint. The prediction is that short-lived water retention may improve hydration without a corresponding persistent recovery signal. Additional, reproducible improvement in the matched recovery endpoint would better support a contribution beyond short-term moisturization.

Evidence layer

What would change the judgment?

Inspect effect sizes, uncertainty and assay sensitivity when results are not significant. Wide intervals leave the question unresolved. A prespecified equivalence margin is needed before narrowing an explanation on the basis of equivalence. These studies do not establish a specific commercial product’s repair mechanism or a treatment recommendation.
